Please read through the information first before submitting your inquiry to employeehealthquestions@usuhs.edu. FAQs

Q: I am not able to return 21 days after my first shot. Am I able to postpone my second shot?

A: The CDC currently recommends the second dose to be given at 21 after the first dose. If you are unable to follow the recommended interval, the second dose may be scheduled to be given up to 6 weeks (42 days) after the first dose.

Q: Do I need to have an appointment in order to get the shot?

A: Yes, everyone must sign up for an appointment date using the registration tool. Please do not just show up without a confirmed appointment.

Q: Am I able to register a family member for the vaccine?

A: Unfortunately, at this time eligibility criteria does not include family members.

Q: Do I need to have a CAC in order to register for the vaccine?

A: If you have a base access identification card (e.g., active CAC or DBIDS), that should enable you to participate in the vaccination program.

Q: I'm on the waitlist. Should I show up?

A: No. The waitlist automatically promotes people to the confirmed list in the event that someone with a confirmed reservation cancels. You should not plan to attend a vaccination session if you do not have a confirmed reservation.

Q. Is there a time I need to arrive?

A. No. You may be vaccinated any time between 0700-1700, however we recommend you arrive no later than 1600 to complete the entire process.
