Optional. If you were issued an in-processing form instructing you to visit the USU Learning Resource Center (LRC) for a signature, you have two options.

• Option A: Electronic Form
  If you have an electronic form, please email the form to lrc.services@usuhs.edu. LRC staff will digitally sign the form and return the signed copy to you.
   
• Option B: Paper Form
  If you have a paper form, please bring the form to the LRC Circulation Desk for staff to sign.
